How Can I Get Copies of the Divorce Forms?

If you are in need of Michigan reproducible divorce forms, we have two binders that people can make copies from in the library, behind the help desk.  Until we are open, read through the following form names to let us know what you would like printed off to be picked up via curbside OR scanned to an e-mail. 

According to clerks at the courthouse, in order to start and finish a divorce case you will need the following forms (at a minimum--there may be more needed depending on your individual case):

Without Minor Children:

  • Summons and Complaint
  • Complaint for Divorce
  • Default
  • Motion to Enter Default Judgment of Divorce
  • Judgment of Divorce
     

With Minor Children:

  • Summons and Complaint
  • Complaint for Divorce
  • Uniform Child Custody Jurisdiction Enforcement Act Affidavit

If you are using Friend of the Court, you will also nee the following to start a case:

  • Verified Statement and Application for IV-D Service

To finish the divorce case you will need the following forms:

  • Default
  • Motion to Enter Default Judgment of Divorce
  • Judgment of Divorce

If you are using Friend of the Court, you will also need the following to finish a case:

  • Uniform Child Support Order

NOTE: There may be additional forms that you will need depending on your individual case.
